The names and signatures
of the account holders should be exactly in
the same order as mentioned on the certificates
to be dematerialized.
All communications shall
be sent at the correspondent address of the
sole or first holder only.
In case of additional signatures,
separate annexure should be attached to the
application form.
A Magistrate or a Notary
Public or a Special Executive Magistrate must
attest thumb impressions and signatures other
than in English or in Hindi or any of the
other languages not contained in the 8th
Schedule of the Constitution of India.
In case the applicant is
unable to obtain introduction, the applicant
should furnish along with the application
form a copy of his Passport, Ration Card,
PAN Card, Voters Id Card, bank attestation
or any other such identification document
to the satisfaction of the DP (depository
participant).
The DP Client Agreement
should be on a stamp paper. The stamp paper
charges vary from state to state and hence,
care should be taken to get the appropriate
value of stamp paper.
As minor accounts are to
be operated by the guardian, the guardian
should sign across the photograph of the minor.
Introduction details are
a must and should be clearly mentioned in
the form.
MICR code in bank details
is a must and should be clearly mentioned
in the form.
Shares in maiden name may
be dematerialized in the account opened in
post marriage name. Attested copy of Marriage
Certificate is required to be sent along with
DRF.
